The Safety Committee Secretary shall present the facts of the case and open the review to the Committee for discussion. E. After appropriate discussion, a vote of the members, except the Safety Committee Secretary shall be taken to determine if the incident was preventable based upon an informed analysis of the facts as presented. The Safety Committee Secretary shall vote only if there is a tie vote of the Committee. F. The Safety Committee Secretary shall make a written report of the Accident Review and report the Committee’s findings to the Department Director or Supervisor. IX. SAFETY AWARD CRITERIA 9-01 Purpose To recognize outstanding employees who perform and accomplish their duties and tasks of the department safely. This will translate to reduction of costs associated with accidents, stabilize the work force, improve employee morale and safe working conditions, and improve customer satisfaction. 9-02 Responsibility The immediate supervisor is responsible for enforcing safety practices in his/her department. Every crewmember is responsible for the safety of his/her fellow employees to emphasize teamwork among crewmembers. 9-03 Safety Award The Safety Committee will review all accident reports throughout the year and determine if the accidents are preventable or non-preventable and implement an award system. This would be based on the preventable accidents accumulated during the year. The Safety Committee will take an average of the amount of preventable accidents in the past three years for the department and that will be the goal of department employees to stay under to be eligible for a monetary award. Those employees who have a preventable accident will not receive an award. If total preventable accidents are over the goal for the year, employees for that department will not receive an award. A 3 year average of preventable accidents for the entire City will also be calculated. If preventable accidents go over the
